Somerville Health Foundation Request for Preventative Health Proposals

The Somerville Health Foundation, part of Cambridge Health Alliance, serves to promote and support educational, research and outreach programs that improve primary care and preventive health services for residents of Somerville. The foundation is soliciting proposals from 501(c) 3 organizations for projects that will support the provision of primary care and preventive health services accessible to the residents of Somerville.

The current priorities for funding support are programs or projects with an emphasis on root causes of disease, health equity, health disparities, systemic racism and serving vulnerable populations across all ages among Somerville residents. Projects aimed at addressing inequities caused by COVID-19 will also be considered.
